A membership agreement between you and your provider — no insurance company in the middle.

The Model

## Health care without the insurance maze

Direct Primary Care is a membership health plan that removes insurance company restrictions. It is a straightforward agreement between you and your provider that gives you direct contact — in-person appointments within one to two days, virtual visits, telephone consults, and even text messages.

We believe in evidence-based medicine, while staying clear-eyed about the limits insurance companies place on care. Because we contract directly with labs, imaging centers, and pharmacies, we obtain most common tests and medications at a discount and pass those savings straight on to you.

Membership Costs

## Simple, predictable monthly pricing

Discounts are available for payments made at three months, six months, and one year.

-   Children 0–18 — $50.00 per month 
-   Adults 19–84 years — $80.00 per month 
-   Adults 85 plus — $105.00 per month 
-   Family of four (two adults, two children under 18) — $210.00 per month 

Why Membership Works

## A small panel meansmore of your provider. 

Most insurance-based patients wait more than two weeks to be seen. Our members are usually in the room within one to two days — or answered by text the same afternoon.

What's Included

## Unhurried care, all year round

Every membership includes one yearly physical with a full wellness laboratory panel, unlimited office visits, and common in-office labs including strep screens, urine analysis, and flu tests.

You also get direct provider access with no call centers or triage nurses, plus discounts on the additional labs needed to monitor chronic conditions.

We follow the recommended screening guidelines of the US Preventive Services Task Force and standard CDC vaccination guidance for both children and adults.

Colorado state law prohibits Direct Primary Care providers from contracting directly with Medicaid patients for services covered by Medicaid. This provider has also opted out of Medicare, meaning neither provider nor client can bill Medicare for services rendered at our office.
